 A New Quality Choice in Aged Care

This exciting new service from Montefiore has been created to respond to the needs of the ageing community; with accommodation in aged care facilities at a premium, the demand for our professional services and support at home has grown.

 

Because we offer the widest range of professional services we can tailor a care package to meet the needs of the individual.

 

What is Montefiore Home Care?

Montefiore Home Care is a 24 hour private in home professionally home care program that provides individual solutions to support frail aged people and people with disabilities in our community to remain at home.  The Montefiore Home Care team will coordinate a range of services to meet your individual needs, whilst allowing you to maintain a level of independence and flexibility.

 

 

How does it work?

The Home Care Manager will conduct a full assessment in your own home with you or if needed your carer or advocate. This assessment will review your needs and match those to the potential services we can provide for you.  Following the assessment the case manager will offer you a plan that meets your needs. If you agree to this plan of care we will develop a service agreement to reflect it. All details received during the assessment will remain on file and kept confidential.

 

What the program offers

In the initial stages of the program we will be providing:

 

·        Post Hospital care (transitional care from hospital - physio, wound management etc) 

·        Home Respite services - emergency and planned

·        Nursing Care such as wound management, medication management etc

·        Professional support services such as physiotherapy, rehabilitation podiatry, OT assessment and dietician reviews

·        Falls prevention program
·        Hydrotherapy - therapeutic and excercise programs          

·        Holocaust survivor support services

·        Culturally & linguistically diverse support

·        Support Services when considering Residential care admission

·        Live in care for short term arrangements

·        Holiday Support

·        Domestic Support Services

  

Fees

The costs of the program will be individually determined with either the client or their person responsible. Clients who are financially disadvantaged will be reviewed in line with their required service needs.
