How does it work?
Montefiore HomeCare begins with meeting one of our care professionals.
Step One
Our HomeCare team conducts a comprehensive assessment of your care needs in your own home including assessing your physical, emotional, diversional, recreational and rehabilitation needs. Our first assessment is complimentary.
Step Two
We then match your needs with the services and work with you and your family on developing a care plan.
Step Three
We develop a service agreement to reflect the care plan once you are happy with it. All details received during the assessment will remain on file and kept confidential.
